Fraiche Medspa is dedicated to delivering exceptional aesthetic care to our patients. Our office Policies are carefully crafted to ensure a truly gratifying experience during your visit with us.

STAFF

All Staff are licensed and trained.

BOOKING APPOINTMENTS

To secure your appointment, a valid credit card must be provided during the booking process. If you cancel the appointment with less than 24 hours notice, a nonrefundable fee of $50 will be charged to your credit card.

PUNCTUALITY

Please arrive 10 minutes prior to your scheduled appointment time. When booking services, the time that is allocated is exclusively for your treatment. If you are late for your appointment that may shorten your treatment time.

PAYMENT OPTIONS

Payment for all medical spa procedures is due at the time of the treatment. For specially packaged or grouped treatments, payment for the entire package is due at the time of the first scheduled treatment. We provide several payment options which may be used individually or combined according to your desires:

CASH OR DEBIT CARD: local bank card, or cash

MAJOR CREDIT CARDS: VISA, MasterCard, and American Express

THIRD PARTY FINANCING: A special line of credit through outside companies for financing medical procedures (Cherry & CareCredit)

CANCELLATION AND REFUNDS

We understand that a situation may arise that could force you to cancel or postpone your treatment. Please understand that such changes affect not only our staff but our other patients as well, and we, therefore, request your courtesy and concern. If you need to cancel your appointment, please allow 24 hours to notify us of the cancellation. Should we receive less than 24 hours of notification, or should you fail to keep your scheduled appointment, you authorize Fraiche Medspa to charge a fee of $50 to your method of payment that is stored on file for the missed visit.

THERE CAN BE NO REFUNDS FOR SERVICES ALREADY PROVIDED.

In the event a package or series of treatments has begun, these services will be considered to have been rendered even though the full package or series may not have been completed. Should you wish to discontinue your treatment in the midst of a package or series, credit for the pro rata share of unused treatments is used to purchase other treatments or products of comparable value to the credit offered by Fraiche Medspa. The undersigned authorizes Fraiche Medspa to charge his/her credit/debit card for the fees described above. The undersigned understands that his/her information will be saved to his/her file for future transactions. The undersigned understands that this authorization will remain in effect until he/she cancels it in writing and agrees to notify the merchant in writing of any changes in the account information or termination of this authorization at least 15 days prior to the next billing date if applicable.

REVISIONAL TREATMENT OR TREATMENT OF COMPLICATIONS

The practice of medicine and surgery is not an exact science, and medical spa treatments are the practice of medicine. Although good results are anticipated, there can be no guarantee or warranty, expressed or implied, by anyone as to the actual results you may get. Occasionally additional treatments and/or treatment for problems or complications may be required. These could result in additional charges for which you may be responsible. Your insurance, if you have it, may or may not cover the expenses related to actual complications or other medically related problems arising out of treatment at Fraiche Medspa.

* Please note that medical insurance is not accepted at Fraiche Medspa.

GIFT CERTIFICATES

Gift Cards for any treatment, package, or product may be purchased in the office.

CELL PHONE USE, VIDEOS, AND PHOTOS

To ensure the privacy of all our patients in compliance with HIPAA regulations, recording videos or taking photos in the waiting areas is strictly prohibited.
